In plain language: This explains how we may use your child's health information and how you can access it. We protect it under HIPAA. You can see it, correct it, and know who we've shared it with.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOUR CHILD MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment

We are required by law to maintain the privacy of Protected Health Information (PHI), give you this notice of our duties and privacy practices, and follow the terms of the notice currently in effect.

How we may use and disclose health information

For treatment

To provide and coordinate your child's ABA therapy — e.g., sharing information among your child's BCBA, RBTs, and care team.

For payment

To verify benefits, obtain prior authorization, and bill for services — e.g., sending necessary clinical information to your insurer.

For health care operations

To run our practice, improve quality of care, train staff, and conduct administrative activities.

Other permitted or required disclosures

Uses requiring your written authorization

Most uses not described above require your written authorization, including most marketing and any sale of PHI. You may revoke an authorization in writing at any time, except where we've already relied on it.

Your rights

Changes

We may change this notice and make new terms effective for all PHI we maintain. The current notice is posted at strideb.com and available at our clinic.

Complaints

If you believe your privacy rights were violated, contact us below or the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
